About the opportunity

Uniting is currently undergoing an exciting transformational journey and we welcome you to join us . Uniting Wyndham has an exciting opportunity for a passionate Early Intervention Housing Support Case Manager within our Early intervention team.  This role provides holistic case management to families and single adults over 26yo to sustain or maintain housing. Within this role you will provide case management to the most vulnerable members of our community. The role requires undertaking holistic assessments to determine relevant support, and the provision of  outreach and  support to Wyndham residents.

About you

You will be a passionate case manager with a drive to support clients and families to achieve the best possible outcomes. This  opportunity you will bring a wealth of direct service case management and prior exposure to the housing system platform SHIP  would be advantageous, but not a prerequisite.
